Welcome to PatientWize, a Software as a Service (SaaS) platform dedicated to supporting patients and caregivers with relevant information concerning specific health conditions. By accessing or using our interactive SMS messaging and web application services, you agree to these Terms of Service (TOS). Please read them carefully.

 

1. Acceptance of the Terms

By using our services, you accept and agree to be bound by these TOS and our Privacy Policy. If you do not agree, do not use the service.  By agreeing to these terms, you consent to allow PatientWize, LLC to send you text messages with informational content and/or marketing content. Message/Data rates apply.

 

2. Description of Services

We provide informational services via SMS and our web app. The information provided is for supportive purposes only and is not a substitute for professional medical advice, diagnosis, or treatment.

 

3. User Responsibilities

  • Users must be at least 13 years old to access our services.

  • Users must provide accurate registration details.

  • Users should consult with their healthcare professionals before making decisions based on information received through our services.

We may revise these TOS from time to time. Users will be notified of significant changes via SMS or email. Your continued use after the changes have been made implies your acceptance of the revised terms.

These TOS and any dispute arising out of them will be governed by the laws of Pennsylvania, without reference to its conflict of laws principles.
